Vaccine Distribution at Mattituck High School

1st dose COVID-19 Pfizer vaccine appointments for those ages 12 and over. 

Second dose will be 8/23/2021. 

Please bring identification and proof of age.  Parent or Guardian must be present for those ages 12-17. If are not eligible, you will not be vaccinated.

Please also complete and print your NYS vaccine consent before scheduling an appointment CONSENT FORM LINK  Bring this completed consent to your appointment.  

Please wear a short-sleeved shirt for ease of access to vaccination site. Masks are required as is social distancing.
