ELIGIBILITY CRITERIA:

-YOU MUST BE 18 YEARS OLD to receive the Moderna COVID-19 Vaccine  [Recipients must bring a Photo ID, with a date of birth] 
-You MUST meet ONE of the following:
      -New York Resident OR
      -Work in New York State OR
      -Study in New York State

If you work in New York State, you can bring a letter from an employer or a paystub
If you study in New York State, you can bring a copy of a transcript
